Key Responsibilities\n

• Conduct comprehensive mental health assessments, integrating consideration of co-occurring medical conditions such as autoimmune disorders and chronic pain that impact mental wellness.

• Develop and implement tailored and integrated treatment plans that accommodate both psychiatric and medical needs of patients, leading to successful recovery pathways.
\n
• Facilitate individual and group therapy sessions, employing evidence-based therapeutic methodologies to guide patients in their healing journeys.
\n
• Work closely with physicians and healthcare providers to ensure seamless coordination of care, enhancing treatment outcomes through collaborative approaches.

• Provide immediate crisis intervention and support, guiding patients through mental health emergencies with empathy and professionalism.
\n
• Educate patients on the relationship between mental health and physical health, promoting self-management strategies for improved overall health.

• Uphold rigorous documentation standards, ensuring that all records are kept confidential and compliant with legal and ethical guidelines.

• Engage in interdisciplinary team meetings and case conferences to continuously evaluate patient progress and refine care strategies.

• Stay informed about advancements in integrated care practices, continuously enhancing your skills to better serve our patient population.

Patient Population / Client Focus\n

Our patient population consists of individuals navigating a variety of challenges linked to mental health and co-occurring medical conditions, including substance use disorders and chronic illnesses. You will play a crucial role in helping them to manage their mental health while addressing the broader context of their physical health needs.

Qualifications\n

• A Master’s or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
\n
• Active and valid licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Clinical Social Worker - R Privilege (LCSW-R), Licensed Psychoanalyst, or Psychologist.

• Strong interpersonal and communication skills, with an ability to build rapport and trust with a diverse patient population.

• A commitment to maintaining professional boundaries, confidentiality, and ethical practice.

• Comfort in navigating electronic health record systems and telehealth platforms.

• A collaborative spirit, with a proven track record of thriving in interdisciplinary environments.

Compensation and Benefits\n
