Duties

• Conduct comprehensive assessments of patients’ physical conditions, including mobility, strength, and functional abilities.
• Develop and implement individualized care plans tailored to each patient’s needs, goals, and medical history.
• Deliver targeted physiotherapy interventions such as trigger point therapy, orthopedics-focused treatments, and geriatrics care to optimize recovery outcomes.
• Manage discharge planning processes by coordinating with healthcare teams to ensure smooth transitions from inpatient or outpatient settings to home care.
• Educate patients and their families on exercises, injury prevention strategies, and physiologic concepts to promote self-management.
• Document all patient interactions accurately using Electronic Medical Record (EMR) systems, maintaining detailed records of progress and treatment adjustments.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care across acute care, hospital, outpatient clinics, and community settings.

Experience

• Experience in home health or community-based care preferred.
• Background in inpatient or hospital settings beneficial for acute care decision-making.
• Outpatient/clinic experience in orthopedics, geriatrics, and general rehabilitation strongly valued.
• Strong knowledge of anatomy, physiology, and exercise prescription for patient-specific treatment planning.
• Experience with discharge planning and care coordination for hospital-to-home transitions.
• Proficiency with EMR documentation (Winnolas EMR training provided).
• Ability to deliver safe, effective therapy across diverse patient populations while maintaining evidence-based practice.
