Key Responsibilities\n

• Engage with clients both in virtual settings and occasionally face-to-face when necessary, assisting those with major mental health issues and severe emotional challenges.

• Conduct brief, evidence-based screenings to assess clients' needs, followed by arranging or providing more detailed assessments when warranted.

• Create and implement individualized care plans while ensuring that clients have access to necessary linked services and facilitate information exchange between clients and their families.

• Provide a range of treatment services including individual, group, and family therapy, while also conducting essential crisis interventions as needed.

• Monitor client progress through regular assessments and documentation, ensuring timely and efficient completion of reports and data submission.

• Participate actively in team meetings and committees aimed at improving service delivery and ensuring compliance with organizational ethical standards.

Patient Population / Client Focus\n

This position primarily serves individuals with complex mental health conditions, including those facing end-of-life transitions in a hospice setting. The LCSW will work collaboratively to address the unique psychological, emotional, and social challenges these clients encounter, ensuring culturally competent care that acknowledges and respects diverse backgrounds and beliefs.

Qualifications\n

• A valid LCSW license in Arizona is required.

• Expertise in mental health assessment, diagnosis, and crisis management.

Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to 85,000 annually, commensurate with experience and education.
