Key Responsibilities\n

• Engage in program planning and organization of social work services tailored to the unique needs of hospice patients and their families.

• Maintain direct lines of communication with physicians and healthcare teams to report patient progress, concerns, and any potential issues that may arise during treatment.

• Document clinical assessments and treatment plans comprehensively and accurately, including timely updates of clinical and progress notes in accordance with department policies.

• Participate actively in the multidisciplinary team approach to develop and assess treatment plans, participating in regular case reviews to adjust care strategies based on evolving patient needs.

• Provide crisis intervention and counseling services, employing evidence-based methodologies to assess and treat emotional and behavioral challenges.

• Coordinate with patients’ families and support systems to ensure a comprehensive understanding of treatment options and successful discharge planning, tailored to the psychosocial context of each individual.

Patient Population / Client Focus\n

This position focuses on high-risk patients receiving hospice care, who may encounter significant emotional, social, and environmental hurdles as they navigate their health journeys. Your role as an LICSW will help these individuals address complex issues that may obstruct their access to quality medical care and improve their overall quality of life.

Qualifications\n

• A valid state license as a Licensed Independent Clinical Social Worker (LICSW).

• A Master's degree in Social Work, Counseling, Marriage and Family Therapy, Psychology, or a closely related field.

• A minimum of three years of experience working in an outpatient setting, ideally within palliative care or hospice services.

• Strong proficiency in using electronic medical records, word processing software, and other office programs.
\n
• Exceptional verbal and written communication skills, with an emphasis on teamwork and collaborative care.

Compensation and Benefits\n
