Key Responsibilities\n

As a Licensed Professional Counselor at Pathways Wellness Center, you will be responsible for a wide range of tasks that directly contribute to the treatment and support of our patients. Your daily responsibilities will include:

• Communicating regularly with physicians to report on patient progress, identify needs, and discuss any concerns that may arise during treatment.

• Maintaining accurate and timely records that include clinical documentation and progress notes, ensuring compliance with departmental policies and healthcare regulations.

• Collaborating closely with healthcare team members to develop comprehensive treatment plans, including effective discharge planning based on psychosocial assessments and recognized therapeutic interventions.

• Providing counseling and crisis intervention services, addressing emotional and behavioral issues prevalent among patients.
\n
• Assisting clients with discharge planning while facilitating access to community resources and services through effective care coordination.

• Consulting with family members and support systems to ensure a holistic approach to the client's treatment and successful transition post-discharge.

Patient Population / Client Focus\n

In this role, you will engage with a diverse group of clients, including individuals experiencing various mental health conditions such as anxiety, depression, and trauma-related disorders. Your person-centered approach will ensure that each patient's unique needs are met, promoting their overall mental health and well-being.

Qualifications\n

The successful candidate will possess the following qualifications:

• A Master’s degree in Professional Counseling from an accredited institution.

• Current licensure as a Licensed Professional Counselor (LPC) or equivalent.

• At least 2 years of full-time experience delivering direct mental health services.

• Proficient in utilizing computer systems and software, including Microsoft Office, for documentation and communication purposes.
