This position is designed for remote work, allowing for flexible scheduling to accommodate the differing needs of our staff and clients. Although your primary work will occur outside of a traditional office setting, you will collaborate frequently with team members and healthcare professionals, utilizing digital tools and platforms to ensure seamless communication and support for our clients. Our outpatient office setting focuses on individualized counseling that encourages independence while maintaining a strong support network.

Key Responsibilities\n

• Conduct thorough assessments of patients' mental health needs, designing personalized treatment plans that address their unique circumstances.

• Engage in effective communication with supervisors, caregivers, and other relevant parties to monitor patient progress and adapt treatment as necessary.

• Provide a proactive schedule for patient appointments, ensuring timely follow-ups and home visits to assess progress and provide continuous support.
\n
• Facilitate interdisciplinary team meetings to develop shared strategies for improving patient outcomes and encourage cohesive approaches.
\n
• Document concise and comprehensive monthly and quarterly progress notes that capture patient developments and the effectiveness of implemented strategies.
\n
• Act as a mentor by organizing training and certification sessions for new team members to ensure high standards of care.
\n
• Maintain accurate and organized records in compliance with legal, ethical, and organizational standards to protect client confidentiality.
\n
• Regularly liaise with community resources and support networks to enhance treatment pathways and offer comprehensive care for clients.

• Assist in developing training materials and procedures to enhance staff onboarding and skill development.

Patient Population / Client Focus\n

The Licensed Professional Counselor will work with a diverse range of clientele, including both adults and youth. Your expertise will be essential in addressing various issues such as mental health disorders, substance misuse, trauma, and family dynamics. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field, alongside active and unrestricted licensure as an LPC in the state of Arizona.
