Key Responsibilities\n

• Conduct comprehensive assessments tailored to the individualized needs of each patient.

• Maintain ongoing communication with supervisors, healthcare teams, family members, and guardians regarding patient progress and care plans.

• Provide supervisors with a structured schedule of appointments, home visits, and observations for effective monitoring of patient outcomes.
\n
• Co-ordinate and document home visits to gather data, conduct chart reviews, and facilitate interviews, ensuring every aspect of the patient’s care is duly considered.

• Implement and oversee adherence to program plans, ensuring they align effectively with patient needs and organizational standards.
\n
• Participate actively in interdisciplinary team meetings to share insights, collaborate on strategies, and enhance cross-functional patient care.
\n
• Prepare detailed monthly and quarterly progress notes to document patient developments and therapeutic outcomes.
\n
• Organize training sessions to equip staff and team members with the tools and knowledge necessary to deliver effective program interventions.
\n
• Follow up with patients through interviews and home visits to assess their ongoing support requirements.
\n
• Maintain organized records that are readily accessible to facilitate seamless communication among stakeholders.

• Act as a liaison between the clinic and the broader mental health community to assist with developmental goals and initiatives.

• Contribute to the development of training materials and procedures aimed at enhancing program effectiveness.

The LPC will work with a diverse clientele, including adults, youth, families, and individuals facing a variety of mental health challenges. Qualifications\n

• A Master's or Doctorate degree in Counseling, Psychology, or a closely related field.
