Key Responsibilities\n

• Conduct comprehensive assessments on an ongoing basis to tailor treatment plans to meet the diverse needs of your patients.

• Facilitate effective communication between patients, supervisors, and family members regarding progress and treatment adjustments.

• Manage and organize a tentative schedule for appointments, in-home visits, and assessments to ensure monitoring of patient improvement.
\n
• Carry out in-home visits to conduct essential observations, chart reviews, and engagements with staff for a thorough understanding of client needs.
\n
• Work collaboratively to ensure that all program plans are executed effectively, adapting approaches as necessary.
\n
• Participate in interdisciplinary team meetings to discuss client progress and integrate various perspectives into treatment strategies.
\n
• Document and complete monthly and quarterly progress notes, maintaining accurate records for compliance and reference.
\n
• Actively train and orient new staff on the implementation of program protocols and proper documentation practices to uphold quality standards.

• Provide follow-up support through interviews and home visits, ensuring sustained assistance for clients on their recovery journey.

• Serve as a vital liaison between the clinic and the broader mental health community, representing the organization's goals and fostering partnerships.

• Collaborate with colleagues to develop training materials and procedures aimed at enhancing program effectiveness for our clients.

As an LPC, you will engage with patients experiencing anxiety, depression, trauma, or relationship issues. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related discipline.

• Possession of an active and unrestricted LPC license in the state of New Jersey.
