In this full-time position, you will play a pivotal role in assessing, developing, and implementing tailored physical therapy treatment plans. Work Environment

This position operates within an outpatient office setting, emphasizing the flexibility of home visits as part of our home health team. Your daily work schedule will be Monday to Friday, from 9 AM to 5 PM, allowing for a supportive work-life balance while effectively catering to our patients' needs.

Key Responsibilities

• Conduct comprehensive in-home assessments to evaluate patients' physical conditions, determining their unique needs and developing personalized care plans.
• Administer a variety of therapeutic interventions, including therapeutic exercises, gait training, balance retraining, and skilled manual therapies to promote recovery.
• Educate patients and their caregivers on the treatment plans and home exercise programs essential for ongoing recovery and rehabilitation.
• Collaborate with health care professionals, including physicians and nursing staff, to ensure a cohesive approach to patient care and optimal outcomes.
• Maintain accurate and timely documentation of patient progress and care provided in compliance with Medicare and agency standards.
• Travel to patients' homes within your designated geographical territory, fostering community connections and providing direct care where it is most needed.

As a Physical Therapist at Clinica Recovery, you will primarily serve patients recovering from surgeries, injuries, or chronic conditions that impact their mobility and overall quality of life. Qualifications

• Active and valid licensure as a Physical Therapist in the state of New Jersey is required.
• A Bachelor's degree in Physical Therapy or a Doctor of Physical Therapy (DPT) from an accredited institution.
• Minimum of two years of professional experience in physical therapy is required, with a preference for candidates who have home health experience.
• Possession of a valid driver's license and reliable transportation, paired with a willingness to travel to various patient locations.
• Strong communication, time management, and interpersonal skills, enabling meaningful connections with patients and their families.
• Ability to work independently in order to deliver high-quality care in a home setting.

Preferred Experience

• Experience in home health care services can be an advantage.
• Familiarity with current physical therapy practices and interventions pertaining to a home care environment.

• Attractive annual salary ranging from $110,000 to $125,000, commensurate with experience.
