Work Environment

This role is primarily onsite, allowing Physical Therapists to interact directly with patients in their homes. Key Responsibilities

• Conduct initial assessments of patients' physical conditions, reviewing medical histories and diagnostic studies to develop tailored treatment plans.
• Implement a wide range of manual therapy techniques and therapeutic exercises aimed at restoring mobility and relieving pain, adapting treatments as needed based on patient response.
• Educate patients and their families about recovery strategies, injury prevention, and lifestyle modifications to support ongoing health and wellness.
• Work collaboratively with physicians, surgeons, and other healthcare professionals to coordinate care and ensure the best possible outcomes for patients.
• Maintain detailed and accurate patient records, tracking progress and adjusting treatment plans in response to patients' evolving needs.
• Participate in team meetings to discuss patient progress, treatment effectiveness, and strategies for enhancing service delivery.

Patient Population / Client Focus

You will be working with a diverse array of patients including older adults recovering from surgeries or injuries, individuals with chronic pain, and patients seeking to improve their physical fitness and mobility. Each patient presents unique challenges and potentials, allowing you to employ your clinical expertise to influence their quality of life positively.

Qualifications

• Must hold a Licensed Physical Therapist credential in the state of New York.
• A Bachelor's Degree in Physical Therapy is required; candidates with a Master's or Doctorate in the field are preferred.
• Strong interpersonal skills to communicate treatment plans effectively and motivate patients.

Preferred Experience

• A minimum of 2 years of experience as a Physical Therapist in an outpatient setting is preferred, though highly motivated new graduates may also be considered.
• Experience using electronic health record (EHR) systems for documentation and communication is a significant advantage.
• Previous experience in a home care setting is a plus, demonstrating familiarity with the challenges and rewards of providing care outside of traditional facilities.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000, commensurate with experience.
