Work Environment\n

This position is entirely remote, allowing for flexibility in your work-life balance while serving a vital role in our community health clinic. Your working hours will be Monday to Friday from 9:00 AM to 5:00 PM, ensuring structured support to clients during regular business hours.

Key Responsibilities\n

• Act as a primary liaison between clients, their families, and outpatient healthcare providers, ensuring a seamless flow of information and support.

• Interact with insurance companies to facilitate pre-certification and manage utilization effectively, navigating the complexities of healthcare reimbursement.

• Participate actively in discharge and aftercare planning, coordinating with multidisciplinary teams to set actionable goals and follow-up strategies for clients post-treatment.

• Provide therapeutic meal support and education, modeling a positive approach towards food and nutrition while promoting healthy habits among clients.

• Deliver compassionate, empathetic, and affirming care tailored to each client’s unique circumstances, considering their intersecting identities and backgrounds.

You will work with a diverse range of clients, including older adults, each facing unique challenges that require a nuanced understanding of their personal and social circumstances. Qualifications\n

• Must possess a Master’s degree in Social Work (MSW) from an accredited institution, reflecting a solid academic foundation in clinical practices.

Compensation and Benefits\n
