Key Responsibilities\n

As a Licensed Professional Counselor at Frontier Behavioral & Mental Health Clinic, your role will include:

• Conducting initial patient intakes to assess mental health status, including diagnostic evaluations and functional assessments.

• Collaborating with primary care providers to develop and implement patient treatment plans that align with established guidelines of our Federally Qualified Health Center (FQHC).
\n
• Delivering brief, targeted therapeutic interventions for clients in need of mental health support while advocating for necessary referrals to specialty services when appropriate.
\n
• Monitoring and documenting patient progress regularly, ensuring proactive follow-ups to adapt treatment strategies as needed.
\n
• Creating comprehensive relapse prevention plans for patients at risk, working to maintain stability in their mental health.
\n
• Providing education and self-management strategies to empower patients in their recovery journeys.
\n
• Consulting with clinic management and fellow team members to enhance behavioral health services and improve outcomes.

• Maintaining accurate clinical records and preparing necessary reports to support treatment efficacy and regulatory compliance.

• Participating actively in team meetings to discuss patient cases and collaboration strategies.

Patient Population / Client Focus\n

Qualifications\n

The ideal candidate will possess the following qualifications:
