Key Responsibilities\n

• Conduct thorough diagnoses and treatment for various mental, emotional, and behavioral disorders, consistently adapting care plans based on patient assessments and progress.

• Develop and implement appropriate care strategies by working collaboratively with patients, their families, and a multidisciplinary care team.

• Engage in crisis intervention by assessing urgent patient needs swiftly and providing immediate resource referrals, such as community services and psychiatric support.

• Lead social work consultations, conducting psychosocial assessments to inform treatment decisions and delivering crisis interventions when necessary.

• Educate and coordinate with healthcare providers, insurers, and community organizations to ensure comprehensive care delivery and facilitate timely patient discharges.

• Act as a patient advocate, ensuring effective communication among care teams and addressing the unique needs of patients and their families.

• Monitor cases for potential avoidable delays or complex discharge needs, and collaborate with care leaders to develop optimal solutions.

Qualifications\n

• A Master’s Degree in Social Work, Counseling, Psychology, or a related field from an accredited institution.

• Current licensure as an LCSW, LMFT, LMHC, or Psychologist in the state of New York is required.

Compensation and Benefits\n

The anticipated salary for this position ranges from $90,000 to $100,000 annually, complemented by potential bonuses and additional compensation opportunities. Our innovative approach to management allows you to focus on patient care without the burdens of administrative overhead and insurance complexities. A one-time sign-on bonus of up to $1,000 is also available.
