Key Responsibilities\n

• Engage in consistent communication with healthcare practitioners to discuss patient progress, report any issues, and collaboratively formulate treatment adjustments based on clinical assessments.

• Maintain meticulous documentation of clinical interactions, including detailed progress notes, treatment plans, and evaluations, in compliance with departmental guidelines.

• Collaborate closely with healthcare team members to develop comprehensive treatment and discharge plans that align with each patient's needs and the psychosocial assessments.

• Deliver individualized counseling and crisis intervention services, applying evidence-based therapeutic approaches to assess and treat emotional and behavioral challenges.

• Assist patients in their transition to the community by coordinating care and facilitating access to essential services in partnership with local providers.

• Actively consult with patients' family members or support systems to involve them in the treatment process and discharge planning, ensuring a holistic approach to care.

Qualifications\n
