Key Responsibilities

• Conduct thorough evaluations of patients by reviewing medical histories and performing physical assessments to determine appropriate treatment plans.
• Implement and adapt treatment protocols based on clinical evaluations, patient presentations, and current best practices in rehabilitation.
• Document patient care activities timely and accurately, including establishing baseline metrics, creating focused plans of care, and maintaining detailed daily progress notes.
• Coordinate with other healthcare professionals to modify treatment plans based on changes in patient conditions, facilitating seamless continuity of care.
• Lead patient discharge planning and follow-up procedures, ensuring patients have the knowledge and resources required for ongoing recovery.
• Provide education and support to patients and their families, fostering a collaborative relationship that promotes engagement in the healing process.

Patient Population / Client Focus

Qualifications

Compensation and Benefits

This position offers a competitive salary within the range of USD 100,000 to 120,000 annually, complemented by a robust benefits package.
