Key Responsibilities\n

• Schedule regular meetings with clients to review progress, address challenges, and determine readiness for discharge when appropriate.

• Develop, implement, and monitor individualized, person-centered treatment plans tailored to each client's needs and goals.

• Maintain up-to-date and accurate documentation of client interactions, treatment progress, and follow-up plans, ensuring compliance with confidentiality and legal standards.

• Conduct thorough intake assessments for new clients to determine appropriate treatment strategies and resources.

Qualifications\n
