Key Responsibilities\n

• Conduct comprehensive assessments tailored to each client's unique needs and circumstances, ensuring a compassionate approach to care.

• Maintain regular communication with supervisors, care staff, family members, and relevant stakeholders to monitor and report on client progress effectively.

• Develop and propose a structured schedule that includes appointments, home visits, and observations, facilitating seamless tracking of work progress and client needs.

• Coordinate and conduct home visits as necessary, integrating in-home observations, chart reviews, and data analysis into therapeutic practices.

• Ensure that the implementation of treatment plans is executed effectively, with regular follow-ups to assess ongoing client development.

• Engage actively in Interdisciplinary Team Meetings and other relevant meetings, contributing valuable insights toward client care and program development.

• Compile and complete monthly and quarterly progress notes to document client advancements and facilitate clinical reviews.
\n
• Organize training sessions to enhance staff competence in treatment plan implementation and documentation procedures.
\n
• Provide ongoing follow-up through interviews, home visits, and observations to ensure comprehensive support for clients.

• Act as a liaison with the public and mental health community, supporting the development of mental health initiatives and programs.

• Assist in the crafting of procedural training materials aimed at improving program delivery for clients.

Qualifications\n
