Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing medical histories and performing specific assessments to determine individual needs and establish a comprehensive treatment plan.
• Communicate evaluation findings and tailor treatment plans to meet the goals of both the patient and their family, ensuring that caregivers are actively involved in the rehabilitation process.
• Implement evidence-based treatment protocols and consistently assess patient progress, making adjustments to care as necessary to reflect changes in patient status.
• Collaborate with multidisciplinary healthcare teams to ensure comprehensive patient care, documenting any changes in patient conditions that may necessitate modifications to the treatment plan.
• Manage discharge planning effectively, providing follow-up care and ensuring that patients receive appropriate recommendations for continued progress beyond therapy.
• Maintain accurate and timely documentation of all patient interactions and treatments, ensuring compliance with healthcare regulations and organizational standards.
• Exhibit exceptional time management skills by maintaining an organized schedule, arriving punctually for patient appointments, and participating in community outreach and marketing activities.
• Engage in ongoing professional development opportunities, providing mentorship and guidance to physical therapy students or less experienced colleagues.
• Contribute positively to departmental goals by participating in quality improvement initiatives and promoting patient access to care.

Patient Population / Client Focus

You will be working with a diverse population of patients, including those recovering from orthopedic surgeries, neurological conditions, and age-related mobility challenges. Qualifications

• Earned a Bachelor's Degree in Physical Therapy from an accredited institution.
• Must hold a valid state license for physical therapy in Florida.
• Current American Heart Association's BLS for Healthcare Providers CPR certification.

Preferred Experience

We require a minimum of two years of experience in the physical therapy field, ideally within home care settings or similar environments that emphasize individualized patient care. Familiarity with electronic medical records (EMR) systems and rehabilitation protocols is also preferred.

Compensation and Benefits
