As a hybrid work setting, employees will enjoy the flexibility to conduct counseling sessions either remotely via video conferencing platforms or in-person within our welcoming facility. Key Responsibilities\n

• Provide individual counseling sessions through digital platforms and face-to-face meetings, ensuring clients receive the necessary support for their mental health needs.

• Conduct comprehensive assessments of clients to identify their emotional, social, and environmental needs, utilizing clinical expertise to inform treatment planning.

• Develop, implement, and monitor individualized treatment plans aligned with clients’ goals, using evidence-based interventions to ensure therapeutic efficacy.

• Advocate for clients by connecting them with appropriate resources, community services, and support systems that promote their well-being.

• Collaborate with an interdisciplinary team of healthcare professionals to deliver integrated care, sharing insights and strategies to enhance client outcomes.

• Maintain accurate and confidential client records in compliance with HIPAA regulations while adapting to electronic health record systems efficiently.

Patient Population / Client Focus\n

The patients you will assist are diverse, ranging from individuals facing chronic illness and mental health challenges to those in need of emotional support while navigating life transitions. Many clients may be dealing with grief, loss, and trauma, requiring a sensitive and customized approach to their therapy.

Qualifications\n

• A minimum of a Master’s Degree in Social Work from an accredited institution.

• Current and valid LCSW licensure in the state of Oregon, with an established record free from disciplinary actions.

• In-depth knowledge and demonstrated practice in clinical social work, including skills in assessment, diagnosis, and treatment planning.

• Excellent interpersonal communication skills with the ability to establish rapport with clients from various backgrounds.

• Proficient computer skills and an openness to utilizing new technology platforms for remote work and client management.

Preferred Experience\n

• Prior experience working in hospice, palliative care, or end-of-life counseling is highly preferred.

• Familiarity with trauma-informed care practices and evidence-based treatment modalities, such as cognitive-behavioral therapy or solution-focused therapy.

• Experience in collaborating with healthcare teams and providing consultation to other professionals about client welfare and treatment strategies.

Compensation and Benefits\n
