Key Responsibilities\n

As a Licensed Professional Counselor at Yakima Family Health Services, your responsibilities will include:

• Assisting primary care providers in identifying, treating, and managing mental health and psychosocial issues, serving as an integral member of the patient care team.

• Conducting detailed patient intakes focused on diagnostic and functional evaluations; recommending treatment goals and plans tailored to individual needs.

• Providing brief yet effective interventions for patients requiring mental health support, ensuring timely access to necessary therapies.

• Collaborating with primary care providers to determine the appropriateness of patients for continued care within our facility or referral to specialized mental health services.

• Performing follow-ups to track patient progress and adjust treatment plans as needed based on their response.

• Developing and implementing relapse prevention plans and assisting patients in maintaining stability in their lives.

• Identifying and creating plans for “at-risk” patients to prevent the escalation of mental health issues.

• Coordinating behavioral health services and liaising with external treatment providers when permitted by patients.

• Accurately documenting patient progress and clinical findings within treatment charts, ensuring effective communication with all members of the healthcare team.
