Key Responsibilities\n

• Conduct comprehensive patient intakes and assessments focusing on diagnostic and functional evaluations, subsequently collaborating with primary care providers to create personalized treatment plans.

• Evaluate patient cases to determine the appropriate level of care, advising on referrals to specialty mental health facilities when necessary.

• Develop and implement relapse prevention strategies, assisting patients in maintaining stability and managing their conditions effectively.

• Engage in regular follow-ups with patients to assess their wellbeing and adjust treatment strategies as needed, fostering a responsive and adaptive approach to care.

• Document all patient interactions, treatment progress, and diagnostic information accurately in clinical records, ensuring compliance with healthcare regulations and best practices.

Qualifications\n
