Key Responsibilities\n

• Conduct thorough psychosocial assessments for clients, focusing on their mental health concerns, family dynamics, and available resources, to inform the development of comprehensive care plans.

• Provide individual, family, and group therapy sessions using evidence-based approaches tailored to the hospice setting, including cognitive-behavioral therapy and grief counseling.

• Develop and implement individualized treatment plans that establish clear goals and appropriate interventions aligned with clients' needs.

• Collaborate with interdisciplinary teams comprising physicians, nurses, and other healthcare professionals to ensure holistic and coordinated care for each patient.

• Facilitate therapeutic group sessions, psychoeducation classes, and support groups to provide community-based support, enhancing resilience and coping strategies among clients and families.

• Offer crisis intervention and emotional support during acute situations, assisting clients and their families in navigating complex decisions about care.

• Maintain meticulous and timely documentation that adheres to all ethical and legal requirements, ensuring client confidentiality and compliance with hospice regulations.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at Tranquil Pathways, you will work closely with patients facing serious illnesses, including life-limiting diseases, and their families. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active and unrestricted licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.

Compensation and Benefits\n
