Key Responsibilities\n

• Conduct in-depth mental health assessments and screenings to effectively evaluate the individual needs of hospice patients, crafting personalized treatment plans based on these assessments.

• Provide compassionate and responsive short-term individual counseling services, as well as crisis intervention for patients experiencing acute emotional distress.

• Lead and facilitate therapy groups and workshops, covering important topics such as grief, loss, coping strategies, and self-care techniques aimed at patient well-being.

• Work closely with various hospital departments, including nursing, social services, and community organizations, to coordinate comprehensive care delivery tailored to each patient’s circumstances.

• Maintain accurate, timely, and confidential records of patient interactions while adhering to ethical and legal standards in all documentation.

Qualifications\n

• An active state license as a Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).

• A Master's or doctoral degree in Counseling, Psychology, Social Work, or a related discipline.
