Take your Nurse Practitioner expertise beyond the clinic setting by providing impactful care in patients homes. In this dynamic per diem opportunity, youll conduct home health risk assessments for Medicare Advantage plan members across locations in Mitchell County, NC, with visits potentially occurring in Spruce Pine, Bakersville, and other surrounding areas. Each assessment typically spans about 45 minutes and may vary in focus, encompassing annual health reviews or post-discharge appointments.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Meet a minimal hourly commitment weekly or bi-weekly

Key Responsibilities:

• Perform thorough home health risk assessments for a Medicare Advantage health plan
• Evaluate patients prior/current medical history, health conditions, and medication use
• Educate patients on their medical conditions and prescribed medications
• Identify and document any gaps in care, offering actionable recommendations
• No requirement to devise treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Current CPR (BLS) certification
• National board certification via AANP or ANCC
• Minimum of six months NP experience preferred
• MSN degree from an accredited nursing program
