Key Responsibilities\n

• Collaborate with primary care practitioners to identify, evaluate, and administer treatment for mental health and psychosocial issues, enhancing the primary care team's capacity to provide holistic care.

• Conduct comprehensive patient intakes with a focus on diagnostic and functional evaluations to formulate personalized therapy objectives and actionable treatment recommendations.

• Provide immediate and effective mental health interventions for clients in crisis, ensuring their safety and emotional stability.

• Counsel primary care providers on patient appropriateness for primary care treatments versus referrals for specialized mental health services.

• Perform regular follow-ups to assess treatment efficacy, adjust strategies as necessary, and ensure patients are on track for recovery.

• Work collaboratively with clients in developing robust relapse prevention strategies to support sustained recovery.
\n
• Identify patients at risk for deteriorating health and develop targeted preventive programs tailored to their needs.
\n
• Coordinate mental healthcare delivery among various providers, ensuring continuity of care for patients who consent to additional treatment.

• Maintain accurate and up-to-date documentation of patient progress and care in treatment charts.
\n
• Actively participate in communication with primary care providers about patient needs, treatment adjustments, and collaborative planning steps.

• Empower patients with essential self-management strategies and educational resources, fostering their engagement in health decision-making.

• Consult regularly with clinic management and team members on behavioral health services, exploring avenues for program evaluation and improvement.

• Support patients' adherence to prescribed medical therapies and assist in coping with any treatment-related side effects.

• Uphold the Clinical Professionals' Code of Ethics in all interactions and decision-making processes.

Patient Population / Client Focus\n

Qualifications\n

• Master's Degree in Clinical Counseling or a closely related field is required.

• A valid LPC (Licensed Professional Counselor) license in Pennsylvania is necessary.

• Experience in mental health evaluation, treatment planning, and applying DSM-IV diagnostic criteria.

• Understanding of behavioral health frameworks, family therapy, group therapy, and developmental psychopathology.

• Proficiency in English; Spanish fluency is a plus but not mandatory.

Compensation and Benefits\n

The salary for this role ranges from USD 90,000 to 105,000 per year, commensurate with experience and qualifications.
