Key Responsibilities\n

• Conduct comprehensive patient intakes, focusing on detailed diagnostic and functional evaluations to inform individualized therapy objectives.

• Work collaboratively with primary care providers to effectively identify, treat, and manage various mental health and psychosocial issues, enhancing the overall efficacy of the care team.

• Deliver timely and focused mental health interventions to patients in need of emergency treatment, ensuring their immediate concerns are addressed.

• Maintain open lines of communication with primary care practitioners, providing updates on patient progress and participating in collaborative treatment planning efforts.

• Implement ongoing follow-up evaluations to monitor treatment effectiveness, making necessary adjustments to care plans based on patient feedback and progress.

• Support patients in developing relapse prevention strategies and provide ongoing engagement to help them adhere to treatment plans.

• Document patient interactions, progress, and assessments accurately, adhering to Agency and Federally Qualified Health Center (FQHC) compliance standards.

• Advise on behavioral health services, contribute to program development initiatives, and identify opportunities for outcome evaluations within our clinic.

• Uphold the highest standards of ethics and conduct outlined in the Clinical Professionals' Code of Ethics.

• Perform additional duties as assigned, contributing to the overall mission of Brightways Recovery Plus, Inc.
