This role is situated in a home care setting, allowing therapists to work directly with patients in the comfort of their own environments. Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing their medical history, performing specific assessments, and documenting findings to create tailored treatment plans.

• Engage with patients and their families to clearly explain evaluation results and collaboratively set achievable rehabilitation goals.

• Implement individualized treatment protocols that align with patient presentations and adapt to ongoing changes in their health status.

• Collaborate with multidisciplinary healthcare professionals to optimize patient care, recognizing and documenting any changes in patient conditions that may necessitate adjustments in the treatment plan.

• Oversee discharge planning and ensure follow-up care is organized; maintain precise and timely documentation throughout the patient care process, including evaluations, daily notes, interim summaries, and discharge evaluations.

• Manage time effectively to achieve productivity expectations and maintain an accurate schedule, ensuring timely attendance at meetings and participation in community outreach and marketing initiatives.

• Contribute proactively to department objectives, adhering to regulatory guidelines and fostering a culture of compliance and ethical practice.

• Engage in ongoing professional development by pursuing continuing education opportunities that enhance personal competencies and contribute to the organization's advancement.

• Promote an exceptional patient experience by being flexible and adaptable to individual needs, ensuring high accessibility to care for all patients.

Patient Population / Client Focus

The ideal candidate will work with a diverse group of patients experiencing a range of orthopedic and neurological conditions. This includes post-surgical rehabilitation, chronic pain management, and functional mobility issues. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.

• A state license to practice physical therapy, as required by Florida regulations.
