Key Responsibilities\n

• Ongoing Patient Assessments: Perform thorough assessments of clients’ mental health needs, developing tailored treatment plans that are continuously monitored and updated.

• Inter-Communicative Collaboration: Maintain regular communication with supervisors, healthcare staff, and family members to discuss clients' progress and adjust treatment plans appropriately.

• Scheduling Coordination: Provide a structured schedule for appointments and home visits, ensuring timely follow-ups and engagements with clients requiring additional support.

• Home Visit Coordination: Facilitate and organize home visits to conduct in-depth observations and gather relevant data that informs ongoing treatment strategies.

• Program Implementation Oversight: Supervise the execution of treatment plans, ensuring therapeutic approaches are effectively carried out and compliance is maintained.

• Team Collaboration: Actively participate in interdisciplinary meetings to discuss treatment progress, share insights, and enhance service delivery.

• Documentation: Regularly complete and maintain comprehensive progress notes that detail patient progress and outcomes as part of our commitment to quality service.

• Staff Training and Certification: Organize and deliver training sessions for team members on program implementation and documentation practices to ensure quality care is consistently delivered.

• Continuous Support: Facilitate follow-up interviews and additional visits to ensure continual support and reinforcement of treatment goals.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Current, active, and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Arizona.
