Key Responsibilities

• Conduct thorough evaluations of patients, reviewing medical histories and performing specific assessments to determine suitable treatment plans.
• Engage with patients and their families to explain evaluation findings and collaboratively set rehabilitation goals, ensuring everyone is invested in the care process.
• Implement evidence-based treatment protocols tailored to individual patient needs, utilizing advanced techniques and modalities supported by the latest healthcare advancements.
• Consult with other healthcare professionals to coordinate care, promptly document changes in patients' conditions, and modify treatment plans as necessary.
• Manage discharge planning effectively, ensuring patients understand follow-up care and have access to additional resources post-discharge.
• Maintain accurate documentation including initial evaluations, daily treatment notes, and progress summaries, adhering to regulatory standards and organizational guidelines.
• Demonstrate effective time management to meet productivity expectations while being punctual for meetings and adhering to a structured schedule.
• Contribute actively to departmental objectives and initiatives, ensuring compliance with accreditation regulations and participating in the development of best practice procedures.
• Pursue continuing education opportunities to support personal and professional development, and serve as a clinical instructor, guiding students under your mentorship.
• Engage in community outreach efforts, promoting the benefits of physical therapy and expanding access to care within the local population.

Patient Population / Client Focus

You will be serving a diverse patient population that includes individuals recovering from surgeries, those suffering from chronic pain conditions, and sports injury rehabilitation. Our facility is committed to addressing a wide range of therapeutic needs, ensuring that each patient receives personalized and holistic care throughout their recovery journey.

Qualifications

• Bachelor's Degree in Physical Therapy or a related field.
• Active and valid Physical Therapy license in New Jersey.
• Current certification in Basic Life Support (BLS) from the American Heart Association.

Preferred Experience

A minimum of 2 years of relevant experience in a clinical setting is preferred. Candidates with experience in outpatient physical therapy and a demonstrated ability to implement patient-centered care will be highly regarded.
