Key Responsibilities\n

• Conduct thorough psychosocial assessments by interacting with patients, reviewing medical records, and gathering pertinent information to evaluate emotional, social, and environmental factors affecting recovery.

• Provide counseling and emotional support to patients and their families, focusing on adjustment issues related to illness, injury, or life transitions.

• Collaborate with the rehabilitation team to create individualized treatment plans that address both the psychological and social needs of patients, ensuring an integrated approach to care.

• Facilitate access to community resources, including support groups, vocational rehabilitation services, and housing assistance programs, empowering patients to rebuild their lives.

• Offer education and guidance to patients and families about post-rehabilitation services and community resources available for continued support.

• Participate actively in discharge planning, aiding in smooth transitions to home and other care settings while ensuring continuity of care.

Your work will primarily focus on patients in hospice care dealing with terminal illnesses, requiring specialized support to address their emotional and psychological needs. Qualifications\n

• A Master's degree in Social Work from an accredited program.

Compensation and Benefits\n
