Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to evaluate the needs of each patient, tailoring treatment plans accordingly.

• Maintain effective communication with supervisors, direct care team members, family members, and other support individuals to ensure continuity of care and to monitor individual progress.
\n
• Prepare and submit a tentative schedule for patient appointments, home visits, and necessary observations to track treatment effectiveness.
\n
• Organize and coordinate home visits as required, performing in-home observations, reviewing charts, conducting staff interviews, and analyzing pertinent data.
\n
• Implement and monitor program plans to guarantee efficacy, performing regular check-ins and adjustments based on client feedback and progress.
\n
• Participate in interdisciplinary team meetings to discuss case updates, strategies, and collaboration opportunities.
\n
• Document and complete monthly and quarterly progress notes, maintaining accurate and comprehensive records for all clients.
\n
• Lead training sessions for staff and team members to ensure proficiency in the execution of program plans and associated documentation practices.
\n
• Provide follow-up services, including interviews and home visits, to ensure ongoing support and address any evolving needs of individuals.

• Act as a liaison within the public and mental health community to support the development goals of both clients and the organization.

• Assist in the development of training materials and procedures aimed at enhancing program effectiveness for individuals.

In this role, you will work with diverse populations, including both adults and youth facing various mental health challenges. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a relevant field is required.

• Possession of active and unrestricted licensure as a Professional Counselor in the state of Arizona.

• Specialized training or experience in family therapy, couples counseling, trauma-informed care, or substance misuse is highly desirable.
