Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate the emotional, social, and financial needs of patients and their families.

• Develop and implement individualized care plans, including discharge planning and coordination of community resources for post-discharge support.

• Provide individual and group counseling sessions to address topics such as grief, adjustment issues, and coping strategies.

• Engage with interdisciplinary teams, including nurses, physicians, and spiritual care providers, to optimize patient care and outcomes.

• Deliver crisis intervention and support services to patients and family members in emotional distress, particularly during critical healthcare decisions.

• Maintain accurate documentation of patient interactions, assessments, and treatment plans to comply with legal and organizational standards.

• Participate in hospital committees focused on enhancing social work practices and the quality of care offered at our hospice facility.

Patient Population / Client Focus\n

The focus of this role is on patients receiving hospice care, many of whom face complex emotional challenges during end-of-life transitions. Qualifications\n

• Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n
