Key Responsibilities\n

• Conduct comprehensive assessments of clients to determine mental health needs and tailor treatment plans accordingly.

• Maintain open lines of communication among supervisory staff, direct care providers, family members, and other stakeholders regarding client progress and care coordination.

• Provide a structured schedule to supervisors for appointments, home visits, and necessary observations to track client progress effectively.

• Coordinate home visits to conduct in-home observations and assessments, ensuring a thorough understanding of clients’ needs to inform treatment.

• Implement and monitor treatment plans to ensure they are effective and met with the clients’ goals.

• Participate actively in interdisciplinary team meetings to discuss cases and collaborate on client care strategies.

• Complete thorough monthly and quarterly progress notes to document client advancements and areas needing attention.

• Organize training sessions to equip staff and team members with the necessary skills for effective implementation of treatment plans and proper documentation practices.

• Follow up with clients through interviews, home visits, and observations, ensuring ongoing support to address evolving needs.

• Maintain accurate and organized records to ensure information is accessible to relevant parties involved in client care.

Qualifications\n
