Work Environment\n

This full-time position is a remote role, designed to offer flexibility while ensuring you remain fully engaged with your patients and the interdisciplinary team. While you will primarily work from home, regular virtual meetings and collaborative sessions with other healthcare professionals will help maintain continuity and quality of care. The hours of work are Monday through Friday, from 9:00 AM to 5:00 PM, allowing for a work-life balance while delivering impactful services.

Key Responsibilities\n

• Provide psychotherapy and essential mental health services via telehealth platforms to patients experiencing conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD).

• Conduct thorough assessments to evaluate patients’ emotional, behavioral, and social functioning, ensuring individualized care plans.

• Develop and implement evidence-based treatment plans, tailored specifically to meet the unique needs of each patient within the hospice setting.

• Collaborate with other healthcare professionals, including physicians and nurse practitioners, to ensure a well-rounded approach to patient care that emphasizes coordination and teamwork.

• Monitor patient progress regularly, adjusting treatment approaches based on feedback from patients and interdisciplinary healthcare team members.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
