Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to deeply understand each client’s mental health issues, strengths, and resources.

• Provide individual, family, and group therapy utilizing evidence-based modalities like cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed approaches, tailored to the unique circumstances of hospice care.

• Develop and execute individualized treatment plans that align with the therapeutic goals of clients and engage them in their own care process.

• Collaborate closely with a skilled team, including psychiatrists and psychologists, to ensure holistic and coordinated treatment plans are in place.

• Offer crisis intervention and develop safety plans, especially for clients facing acute mental health challenges.

• Maintain precise and timely documentation in compliance with ethical and legal standards and framework.

• Facilitate group therapy sessions and psychoeducational workshops aimed at empowering clients and families through the challenges associated with hospice care.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

The competitive salary for this position ranges from $80,000 to $90,000 annually, in addition to a sign-on bonus.
