Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews and observations, as well as a comprehensive review of medical records to evaluate the emotional, social, and environmental factors affecting patient recovery.

• Provide active counseling and emotional support to patients and their families, efficiently addressing adjustment issues, anxiety, stress, and other mental health concerns.

• Collaborate with multidisciplinary team members to develop individualized, goal-oriented treatment plans that address the unique psychosocial needs of each patient.

• Assist patients in navigating and accessing essential community resources, including support groups, vocational rehabilitation services, and housing assistance tailored to their specific needs.

• Educate patients and their families about community resources and post-rehabilitation services to promote sustained recovery and independence.

• Participate actively in discharge planning processes, ensuring smooth transitions to home or other care settings.

Patient Population / Client Focus\n

Your primary focus will be on patients in the detoxification process who may be dealing with substance use disorders and require emotional and social support during a critical phase of recovery. You will also work with individuals facing various rehabilitation challenges, such as those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders.
