This is a remote position based at our hospice center located in Ravena, New York. Key Responsibilities\n

As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Conducting comprehensive evaluations and assessments to ascertain the needs of children and families.

• Participating actively in the development and implementation of Individualized Family Service Plans (IFSPs), tailoring interventions that cater specifically to the child’s developmental requirements.

• Monitoring and reporting on each child’s progress towards their developmental milestones within a collaborative team framework.
\n
• Providing hands-on assistance and support to families to empower them in advocating for their child’s needs.
\n
• Utilizing evidence-based practices and the Natural Learning Environment Practices to ensure effective family engagement and learning.
\n
• Maintaining compliance with all federal and state regulations while safeguarding the well-being of both clients and staff.
\n
• Building nurturing relationships with children, families, and colleagues to create a supportive atmosphere conducive to individual growth.
\n
• Educating families by providing knowledge and skills necessary for their child's development through coaching methods in natural settings.
\n
• Documenting all interactions and progress in accordance with agency policies.
\n

Patient Population / Client Focus\n

This position focuses primarily on infants and toddlers with disabilities or developmental delays, with a commitment to early intervention tactics aimed at fostering their physical, cognitive, social/emotional, and communication skills. Your work will have a direct impact on the children's ability to thrive within their families and communities.

Qualifications\n

• A Master's Degree in Social Work (MSW) or a Doctorate in Social Work (DSW).

• Active LCSW license issued by the Department of Public Health.
\n
• A valid driver’s license is required for occasional visits to families.
\n
• Proficient in adapting communication and learning strategies to meet each child's unique needs.

• Exceptional organizational and time management skills essential for managing a varied caseload.

• Strong interpersonal and communication abilities to collaborate effectively with a multidisciplinary team.

Preferred Experience\n

• A minimum of one year experience working directly with children in early intervention settings is highly desired.

• Experience working in hospice care or high-stress environments is advantageous.
