Key Responsibilities\n

• Participate in program planning and organization of social services aimed at improving patient care delivery.

• Regularly communicate with physicians and other healthcare professionals to report on patient progress, address needs, and discuss concerns or potential problems.

• Maintain accurate and timely clinical records, including progress notes, in accordance with departmental guidelines and policies.

• Collaborate with the health care team to develop comprehensive treatment plans based on psychosocial assessments and accepted social work practices.

• Provide individual and group counseling sessions, facilitating crisis interventions that address both emotional and behavioral issues.

• Consult with family members and support systems to develop effective discharge plans, ensuring continuity of care.

Qualifications\n
