Key Responsibilities\n

• Conduct comprehensive psychosocial assessments and collaborate with healthcare teams to develop tailored care plans for patients.

• Provide individual and group counseling for patients, focusing on emotional support, coping strategies, and adjustment to illness.

• Coordinate closely with community organizations to arrange appropriate post-discharge services, ensuring continuity of care.

• Offer education and emotional support to patients and families, guiding them through treatment options and available resources.

• Participate actively in hospital committees, sharing social work insights and contributing to team-based initiatives.

• Deliver crisis intervention services, providing immediate support to patients and families facing urgent psychosocial issues.

• Maintain thorough and timely documentation of patient interactions in compliance with hospital policies and legal requirements.

• Engage in regular interdisciplinary team meetings to discuss patient progress and optimize care strategies for improved outcomes.

Patient Population / Client Focus\n

You will primarily serve patients receiving hospice care, including individuals with terminal illnesses and their families. Your role will involve addressing emotional, social, and spiritual needs, assisting patients in navigating their treatment plans and end-of-life decisions. By providing tailored support, you will enhance the quality of life for patients and foster a trusting relationship with their families during challenging times.

Qualifications\n

• Master's degree in Social Work from an accredited program.

• Active and unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
