Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the psychological, social, and emotional needs of patients enrolled in hospice care.

• Develop, implement, and monitor personalized treatment plans that are tailored to assist patients and their family members navigate the unique challenges associated with end-of-life issues.

• Utilize evidence-based, solution-focused interventions to facilitate healing discussions, emotional regulation, and grief management, ensuring the well-being of both patients and their families.

• Maintain accurate and detailed clinical records, ensuring confidentiality, compliance with state regulations, and alignment with professional ethics.

Qualifications\n

Compensation and Benefits\n
