Key Responsibilities\n

As a Licensed Clinical Social Worker at Keck Health Care Center, your responsibilities will include:

• Conducting comprehensive bio-psychosocial assessments for mental health, substance abuse, and concurrent medical needs, tailored to the primary care context.

• Providing direct behavioral health services to individuals, couples, and families, including intake assessments, counseling sessions, treatment planning, crisis intervention, and referrals to addiction treatment services as necessary.

• Maintaining accurate and detailed electronic documentation of patient encounters, including mental health evaluations, progress notes, and suicide risk assessments.

• Acting as a liaison for patients, offering social work support and facilitating access to community resources, such as medication assistance programs.

• Collaborating with Quality Improvement teams to contribute to data collection initiatives that enhance service delivery and patient outcomes.

Patient Population / Client Focus\n

You will primarily work with patients receiving hospice care, who may be experiencing various mental health challenges related to their medical conditions. This population often includes individuals and families dealing with end-of-life issues, requiring a sensitive and empathetic approach. Qualifications\n

To be considered for this role, candidates must meet the following qualifications:

Compensation and Benefits\n
