Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments to evaluate the mental health, substance abuse, and medical needs of patients, ensuring culturally appropriate and age-appropriate approaches.

• Deliver a wide range of behavioral health services, including individual and group counseling, crisis intervention, and treatment planning, to empower patients and help them achieve their therapeutic goals.

• Maintain comprehensive and accurate electronic documentation of assessments, treatment plans, and progress notes, playing a pivotal role in the overall patient care continuum.
\n
• Act as a liaison, offering social work support and connecting patients with local resources and community agencies, such as medication assistance programs, enhancing their access to needed services.

• Collaborate with Quality Improvement teams to contribute to data collection initiatives aimed at improving service delivery and patient outcomes.

Patient Population / Client Focus\n

This position primarily serves patients with complex needs in a hospice setting, requiring sensitivity to grief, loss, and end-of-life issues. You will be working with individuals and families navigating difficult life transitions, emphasizing a compassionate and empathetic approach to support their emotional and psychological health.

Qualifications\n

• Current and valid LCSW license in the state of California.

• A Master’s degree in Social Work from an accredited institution.

• A minimum of three (3) years of post-licensure experience in a clinical or healthcare setting.

Compensation and Benefits\n

We offer a competitive salary in the range of USD 115,000 to 130,000 per year, commensurate with experience. Our benefits package includes:
