Key Responsibilities\n

• Conduct thorough, culturally-responsive bio-psychosocial assessments for patients with varying mental health, substance abuse, and medical needs, ensuring comprehensive care within the primary care context.

• Deliver behavioral health services which include individual, couples, and family therapy, as well as crisis intervention and treatment planning tailored to meet the unique challenges faced by each client.

• Act as a liaison between patients and local community resources, providing essential referral services for medication assistance and additional support programs.

• Collaborate with Quality Improvement teams to enhance data collection processes, contributing to clinic-wide initiatives aimed at improving patient outcomes and service delivery.

• Create and maintain detailed electronic documentation of all patient interactions, including progress notes, treatment plans, and suicide risk assessments.

Qualifications\n

• Must possess an active Licensed Clinical Social Worker (LCSW) credential valid in California.

Compensation and Benefits\n
