Key Responsibilities\n

As a Licensed Clinical Social Worker, you will play a critical role in supporting mental health consumers by:

• Providing clinical assessments and behavioral health interventions tailored to individual student needs in a school setting.

• Assisting clients in navigating both physical and behavioral healthcare systems to ensure they receive comprehensive and integrated care.
\n
• Educating patients and their families about available resources and services, encouraging active participation in their health and wellness journeys.
\n
• Collaborating effectively with a multi-disciplinary treatment team during meetings, contributing insights and recommendations to enhance patient care plans.
\n
• Timely reporting of crises or unusual incidents to the Medical Director, ensuring that all procedures are followed to maintain patient safety.
\n
• Maintaining organized and detailed documentation for clinical records as well as compliance with federal and state regulations.
\n
• Participating in Quality Improvement programs and fulfilling all required training sessions, supporting the overall effectiveness of health service delivery.
\n
• Utilizing evidence-based screening tools to assess healthcare needs, documenting and reporting findings to inform treatment strategies.
\n
• Working collaboratively with our Center of Excellence Team to support patients dealing with Opioid Use Disorder.

Patient Population / Client Focus\n

The focus of this position will be primarily on school-aged children and adolescents who may be facing various behavioral health challenges. You will work closely with students experiencing emotional disturbance, anxiety, depression, and issues related to trauma and substance use, fostering a supportive and nurturing environment for their wellbeing.

Qualifications\n

• A Master’s degree in Social Work is required, with a current Licensed Clinical Social Worker (LCSW) designation.

• A minimum of 2-3 years of experience in the clinical social work field is essential, with a preference for experience in primary or specialty care settings.

• Strong collaborative skills to work harmoniously with healthcare professionals, social service agencies, and clients.

• Ability to adhere to ethical standards and maintain confidentiality in all aspects of patient care.

• Willingness and ability to travel to all CHDC sites for meetings, workshops, and outreach events as required.

Preferred Experience\n

• Experience in school-based settings or working with pediatric populations is highly desirable.
