Key Responsibilities\n

• Conduct comprehensive assessments to identify the complex social, emotional, and environmental challenges faced by our clients.

• Develop individualized service plans, clearly setting forth goals and interventions, while employing ongoing progress monitoring to assess effectiveness.

• Provide individual and family counseling, employing evidence-based practices tailored to address each client’s unique situation.

• Collaborate with clients to identify and connect them with essential community resources and services, ensuring they have access to the support they need.

• Engage in crisis interventions when necessary, employing risk assessments and safety planning to aid clients in managing acute distress situations.

• Work in partnership with community agencies and organizations to coordinate services and advocate for client needs, contributing to a network of support.

• Participate in outreach activities aimed at raising awareness of agency services within the community, helping to connect more individuals in need to the resources available.

• Contribute to training sessions and team discussions to enhance the overall effectiveness of our mental health services.

Patient Population / Client Focus\n

Your client focus will predominantly include vulnerable populations dealing with significant life challenges. This may encompass individuals and families grappling with issues such as mental health disorders, economic hardship, trauma from domestic violence, substance dependence, and other stressors that can impede their ability to achieve personal goals. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
