Position Overview

Flexibility Plus Rehab, located in beautiful Middletown, New Jersey, is seeking dedicated and skilled Physical Therapists to join our vibrant team. This full-time role is centered around providing exceptional physical therapy services directly in our patients' homes, allowing us to deliver personalized care that promotes recovery and enhances quality of life. Our Physical Therapists will not only focus on rehabilitation but also provide education and support to patients and their families, guiding them through the challenges of recovery.

Work Environment

This position is primarily onsite as you will provide care in the homes of patients located in the surrounding communities of Middletown. Our therapists enjoy a dynamic work environment that emphasizes flexibility, autonomy, and the opportunity to develop meaningful relationships with their patients. Working Monday through Friday from 8 AM to 3 PM, our physical therapists have a structured schedule that allows for a practical work-life balance.

Key Responsibilities

• Conduct thorough evaluations of each patient's physical condition, considering both historical and current factors impacting their health.
• Develop and implement individualized treatment plans tailored to the specific needs and goals of each client, fostering independence and enhancing functional abilities.
• Regularly assess patient progress, modifying treatment plans as necessary to ensure effective rehabilitation.
• Maintain accurate and timely documentation, including SOAP notes and care plans, utilizing our electronic record system to track patient progress and outcomes.
• Educate patients and their families about injuries, treatment plans, and expected outcomes to instill confidence and encourage active participation in the recovery process.
• Collaborate with multidisciplinary team members to ensure holistic care for patients receiving multiple services, enhancing cross-disciplinary outcomes.
• Meet business objectives and minimum utilization goals to support the operational success of Flexibility Plus Rehab.
• Act as a champion for our company culture and values, representing Flexibility Plus Rehab in a professional and empathetic manner in all interactions.

Patient Population / Client Focus

Our experienced Physical Therapists engage with a diverse patient population, including children and adults with various diagnoses, injuries, and recovery needs. You will have the unique opportunity to work with individuals from different backgrounds, tailored to the needs of each patient—notably including those recovering from surgeries, injuries, or managing chronic conditions, thus showcasing your skills in effective patient care across the lifespan.

Qualifications

• A Bachelor's degree from an accredited physical therapy program; a Master's or Doctorate in Physical Therapy (DPT) is preferred.
• Current, valid, and unrestricted licensure as a Physical Therapist in the state of New Jersey.
• Willingness to conduct evaluations and treatments both face-to-face and via telehealth, adapting to the evolving clinical landscape.
• A strong desire to work with a wide-ranging patient demographic, displaying cultural competence and empathy.
• Flexibility and adaptability to navigate the fast-paced changes within home health services and technology.

Compensation and Benefits

Flexibility Plus Rehab offers a competitive salary ranging from USD 100,000 to 120,000 per year, commensurate with experience and qualifications.
