Key Responsibilities\n

• Conduct comprehensive evaluations and psychosocial assessments for seniors and their families, identifying needs and service gaps.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs) tailored to each client.

• Monitor client progress and outcomes as a collaborative member of the multidisciplinary team, adjusting care plans as necessary to ensure optimal support.

• Provide guidance and support to families, enabling them to navigate the challenges associated with aging and end-of-life care.

• Administer evidence-based interventions to address mental health concerns, such as anxiety and depression, commonly experienced by seniors.

• Maintain compliance with all federal, state, and organizational regulations regarding client care and confidentiality.

• Develop and maintain strong rapport with clients, families, and team members to foster a trusting and nurturing environment.

This position primarily serves the senior population within a hospice setting, addressing a range of age-related mental health issues such as grief, loss, adjustment disorders, and end-of-life anxiety. Qualifications\n

• A Master's degree in Social Work (MSW) or a closely related field from an accredited institution is required.
