This is a remote position, enhancing flexibility and ensuring that you can balance your work with personal commitments. Despite the remote nature of the job, you will remain a key member of our clinic team, collaborating regularly with primary care providers and support staff to effectively address the mental health needs of our patients. Your role will involve regular virtual meetings and consultations, where you will share insights and collaborate on treatment plans.

Key Responsibilities\n

• Assist primary care providers in identifying, treating, and managing mental health and psychosocial issues while being an integral part of a collaborative healthcare team.

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and provide tailored recommendations for treatment goals and plans to primary care providers.

• Deliver brief, focused interventions to patients in need of mental health services, ensuring effective and timely support.
\n
• Advise primary care providers on patient referrals to specialty mental health facilities when necessary, ensuring effective care coordination.
\n
• Initiate follow-up communications to assess patient progress and determine if modifications to treatment plans are needed.
\n
• Develop relapse prevention strategies to help patients maintain stable functioning over time.
\n
• Identify patients at risk and create proactive plans to address and prevent a worsening of their condition.
\n
• Monitor and coordinate the delivery of health services in relation to behavioral healthcare, linking with other treatment providers as authorized by the patient.
\n
• Ensure comprehensive documentation of patient progress and diagnostic information in treatment charts to aid in ongoing care.
\n
• Communicate effectively with primary care providers about patient needs, regularly updating them on progress and treatment strategies.
\n
• Assist patients in making healthier lifestyle choices and managing behavioral changes associated with physical disorders.
\n
• Provide educational resources and self-management skills to empower patients in their treatment and recovery journeys.
\n
• Offer consultation to clinic management and team members regarding behavioral services and program evaluation.
\n
• Support patients in complying with medical treatments initiated by primary care providers, including coping strategies for medication side effects.
\n
• Adhere to the Code of Ethical Conduct for Clinical Professionals and carry out additional responsibilities as necessary.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling is required.

• A current and valid LPC (Licensed Professional Counselor) License is essential.

• Proficiency in mental health assessment, treatment planning, and knowledge of DSM-IV diagnoses.

• Strong understanding of psychotherapeutic techniques, family therapy, and methods relevant to child psychotherapy.

• Ability to evaluate care effectiveness and adapt treatment plans based on individual patient needs.

• Knowledgeable in community resources for mental health support and services.

Preferred Experience\n

Compensation and Benefits\n
