Work Environment

This full-time position is performed onsite in patients' homes, creating a unique work experience outside of traditional clinical settings. You will interact directly with patients, caregivers, and multi-disciplinary teams, providing you with the flexibility to manage your schedule while adhering to the highest standards of care.

Key Responsibilities

• Perform comprehensive evaluations of patients in their home environments to assess physical abilities, challenges, and specific needs.
• Design and implement individualized treatment plans that incorporate evidence-based methodologies to improve mobility, strength, and overall well-being.
• Utilize a variety of therapeutic techniques, including manual therapy, therapeutic exercise, and the use of specialized equipment to enhance patient recovery.
• Monitor, document, and communicate patient progress and response to treatment interventions consistently, ensuring all records meet organizational and regulatory standards.
• Reassess and adjust treatment plans based on ongoing evaluations of patient progress and feedback, facilitating adaptive strategies that fit each patient's unique situation.
• Provide education and training to patients, caregivers, and family members about exercises, home modifications, and assistive devices to promote independence and enhance performance in daily activities.
• Stay abreast of the latest developments in physical therapy practices, attending workshops and collaborating with peers to continually refine your skills and techniques.

Patient Population / Client Focus

You will work with a diverse patient population, including children, adults, and seniors, each with varying diagnoses such as post-operative recovery, neurological conditions, orthopedic injuries, and chronic pain syndromes. Your role will be instrumental in not just treating the physical challenges of patients but also addressing their psychological and emotional needs, fostering a holistic approach to wellness.

Qualifications

• A Bachelor's Degree in Physical Therapy or a Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid physical therapy license in the State of Virginia.
• Strong written and verbal communication skills, enabling effective interactions with patients and healthcare professionals.
• Competence in using computers and various software applications for documentation and communication purposes.
• Ability to adapt to variable situations and maintain a flexible schedule in the home care setting.

Preferred Experience

• At least two years of experience as a Physical Therapist, preferably in a home care or outpatient setting.
• Experience working with geriatric populations and knowledge of chronic disease management.
• Familiarity with Medicare regulations and home care documentation requirements.

Compensation and Benefits
