Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families, both upon admission and during follow-up sessions, to identify emotional, social, and physical needs, using robust social work theories and techniques.

• Collaborate with interdisciplinary health care teams to design and implement personalized psychosocial treatment plans, ensuring patients receive the appropriate referrals for specialty and social services.
\n
• Provide assistance to patients and families grappling with the environmental and psychological implications of their illnesses, focusing on helping them navigate complex health systems.
\n
• Independently manage a caseload of clients with varying clinical needs, which may include severe mental health issues, while adapting treatment approaches to support each individual’s unique situation.

• Engage in proactive case finding by identifying patients and families who may benefit from additional psychosocial support; actively participate in team conferences to discuss case strategies and interventions.

• Deliver supportive counseling and therapeutic services, such as grief counseling and crisis intervention, to empower clients and families to handle the challenges arising from illness and treatment transitions.

• Provide psychoeducation to clients and their families, detailing the patient's diagnosis and care options, while facilitating smooth transitions from youth to adult services.

• Contribute to quality assurance initiatives aimed at improving service delivery and patient outcomes, and assist in the creation of policies and procedures relevant to social work.

• Conduct mental health assessments and provide diagnoses as part of comprehensive treatment plans when required.

• Supervise individual or group sessions for MSWs and student interns, offering guidance and support to foster their professional growth.

• Maintain accurate and up-to-date electronic medical records according to departmental regulations, ensuring all reporting is clear and comprehensive.

Qualifications\n

• A valid license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).
